FMLA REPORTING
DESCRIPTION: If you have a reporting requirement under the Family Medical Leave Act (FMLA), you can use the FMLA Reporting utility to generate the required submission files. Pro Fund Accounting can generate submission files for AbsencePro, The Standard, and MetLife.
To access: Click on the Human Resources module then click the FMLA Reporting menu item.
The following window will appear:
Select which file you would like to generate by clicking the AbsencePro, The Standard, or the MetLife button.
For AbsencePro, the following window will appear:
Enter your AbsencePro account number, account name, the filing date, and a list of pay codes separated by a comma you wish to exclude. To create a test submission, click the Create Export Test File button and select where you wish to save the file. To create the official submission, click the Create Export File button and select where you wish to save the file. In both cases, the file name will be automatically generated for you to meet AbsencePro requirements. A success message window will appear upon completion. Click OK.
For The Standard, the following window will appear:
Step 1: Select which type of export you wish to perform - Money Purchase Contribution or 457G Deferred Compensation.
Step 2: Select the payroll you wish to export by using the drop down list.
Step 3: Enter the maximum number of hours to show in the export file OR check the box to Export actual hours worked instead.
Step 4: Enter the number of days an employee needs to work to be eligible for benefits.
Step 5: Enter the deduction codes for the employee contributions.
Step 6: Enter any employee types to always exclude.
Step 7: If you have a money purchase plan, enter the Money Purchase Contribution rate for all employees. Otherwise, if you have a 457G Deferred Compensation plan, enter the 457G Contribution rate for all employees.
Step 8: Click the Export button to bring up a file dialog box. Enter the name of your upload file and select where you want your file to save. Click Save and the utility will create the selected .CSV file for you to submit. A success message window will appear upon completion. Click OK.
For MetLife, the following window will appear:
Enter your UIS customer number, account name, the file date, and a list of pay codes separated by a comma you wish to exclude. Enter the work state. Check the box if This file contains changes only. To create the submission, click the Create Export File button and select where you wish to save the file. The file name will be automatically generated for you to meet MetLife requirements. A success message window will appear upon completion. Click OK.
Click on the X in the upper right corner of the window to close the window.
